The aim of the project is to create regional network of healthcare and social services for people with chronic mental diseases and to create an independent high-quality therapeutic-rehabilitation programme in the conditions of Marianne of Orange's Hospital in Bílá Voda in order to enable clients to integrate and prepare to join the network of social services and to direct them towards independence. The main purpose is to enhance the quality of the programme resulting in most clients completing it and to interconnect with the programmes of partner organizations facilitating the involvement of clients into aftercare and increasing the remaining in the network. We want to achieve the decrease in recurring hospitalization due to deteriorating health and social state of a client outside the premises of the hospital due to low social and rehabilitation support. We expect that integrating a client into the network and providing high-quality treatment will help to extend the time of the compensation of clients´ condition and improving their lives.
